Make checks payable to the L.A.B.C.C. All registration forms must contain the signature approval of both parents of the child. REGISTRATIONS WILL NOT BE ACCEPTED WITHOUT THESE APPROVALS. Separated or divorced parents must both approve the registration of their child, as well. Exception will be made in the case of a single parent where a second signature is not available.

HEALTH EXAMINATION INFORMATION AND INSTRUCTIONS:

ALL VACCINATION AND BOOSTER SHOT INFORMATION MUST BE INDICATED, PARTICULARY THE DATE OF YOUR CHILD’S LAST TETANUS VACCINATION. WE WILL NOT ACCEPT YOUR CHILD WITHOUT THIS INFORMATION. PLEASE ENCLOSE A COPY OF YOUR HEALTH INSURANCE IDENTIFICATION CARD(S), FRONT AND BACK, WITH YOUR COMPLETED FORM. The camper’s physician/ provider is to complete the “Physical Examination” section of the form. Also, please note that we are requesting information to expedite treatment in the event of an emergency.

All parents must sign the forms and when applicable where both the physician and parent must sign if your child is taking prescription medicine. PLEASE INSTRUCT YOUR CHILD TO TURN IN ALL MEDICATION, UPON ARRIVAL FOR BUS LOADING, TO THE CAMP NURSE FOR DISPENSING AND SAFEKEEPING. ALL PRESCRIPTION MEDICATION MUST BE PRESCRIBED FOR THE CAMPER AND NOT FOR A MEMBER OF THE CAMPER’S FAMILY OR OTHER PERSONS.

Cancellations must be made within 48 hours prior to the opening of the camp session. Please send your registration at once to assure your reservation, as the number of campers that can attend this camp session is limited.

Specific information regarding assembly point, departure date and time, will be provided at a later date.

DEPARTURE: All campers are to assemble at the Nishi Hongwanji Buddhist Temple, 815 E. First Street, Los Angeles rear parking lot located at the northwest corner of First & Vignes Streets no later than 9:30 a.m., Saturday, July 29, 2017. We request your cooperation in observing the following procedures:

• Unload campers and luggage in the parking lot area. Please move your automobile to the outer perimeter of the parking lot immediately after unloading your children’s baggage in order so others may do the same and also, parking space to accommodate busses is needed to load campers and baggage. We request the assistance of fathers and men folks for assistance in loading baggage on the truck and busses.

All campers are to report at the temple gymnasium upon arrival for check in and to pick up bus loading passes...

LUNCH: PLEASE PREPARE A SACK LUNCH FOR YOUR CHILD ON SATURDAY, JULY 29TH. The Campers will eat lunch upon arrival at camp. For security reasons, there will be no stops made by the bus en-route to camp.

RETURN: All campers will return by bus which will depart from camp by 9:30 a.m. Saturday, August 5, 2017 and is scheduled to return to the same Nishi Hongwanji Buddhist Temple North parking lot at approximately 12:00 noon PLEASE MAKE ARRANGEMENTS TO MEET YOUR CHILD AT THIS TIME! We regret there will be no visitors’ day on this date at the campsite due to the early departure time from the campsite on this day. Unannounced appearances by parents wishing to visit their children during the camp session will not be allowed as such visits tend to disrupt our daily camp schedule and program. Parents are allowed only in event of family emergencies. Any other visits must be approved by the camp director only. Generally, family visits with campers are not permitted and discouraged.

GUIDELINES ON WHAT TO BRING: Specific items are listed in the enclosed parents guide. Cell phones, radios, portable CD players, I-pods and games are strictly prohibited. If any of these items are found in the possession of the camper, the item will be confiscated for the duration of the camp session.

2017 REGISTRATION FEES

Registration fees include lodging, meals and transportation for the camp session.

$375
for each of the first 2 children from a single family.
$325 per child
for children in excess of 2 from a single family

For example, fees for 1 child will be $375.00; 2 children from a family will be $750.00; and for 3 from the same family the fee will be $1075.00.  An additional $325.00 fee will be charged for each child thereafter.
